|Who is eligible||A current member of a paid or volunteer emergency medical services agency.|
|Special requirements||The applicant and the applicant's service director shall sign the application form certifying that the applicant is a current member of a paid or volunteer emergency medical services agency. The signatures must be original and notarized.|

Effective July 17, 2013, an owner of an EMS plate may obtain a duplicate of their current numbered EMS plate when their plate is due for replacement. A new application must be completed and submitted with a $25 fee.

Completed applications may be returned to your county treasurer's office or the following address.
Office of Vehicle and Motor Carrier Services
Your plates will be sent to your county treasurer's office and staff will notify you when your plates are available. Plates must be assigned to a vehicle within 90 days or the plates will be voided and no refund issued. Your current plates must be exchanged for your new plates.
