Obtaining a Driver's Record or Accident Report Information


Obtaining a Copy of a Certified Driving Record

Send a written request with your full name, driver license number, birthdate and a check for $5.50 (make your check payable to Treasurer, State of Iowa). Mail your request to:

Office of Driver Services
P.O. Box 9204
Des Moines, Iowa 50306-9204

Include the complete mailing address where the record is to be sent.

Obtaining a Copy of an Accident Report

  • Officer's Report: A copy of the officer's report of an accident costs $4. State law allows these reports to be given only to persons involved in the accident, their attorney, insurance companies or insurance agents. To obtain a copy of the report, send a written request with the date of the accident, time, location, and names and driver license numbers of the drivers involved.
  • Driver's Report: A copy of an accident report you completed for an accident that occurred in 1994 or after is $.50 (fifty cents). For accidents that occurred prior to 1993 is $3.20. To obtain a copy of an accident report that you completed, send a written request with your driver license number, and the date of the accident.

Obtaining a Copy of a Ticket

A copy of a ticket is $.50 (fifty cents). Send your written request with your driver license number, date of the ticket, and a mailing address where the copy should be sent.

Suspension for Failure to Pay a Ticket

A suspension for failure to pay a ticket can be ended only when the Office of Driver Services has received proof that the fine has been paid. The suspension will be shown on your driving record unless the proof from the Court is received before the suspension starts.

If you need more information about the fine, please contact the Clerk of Court in the county listed on the suspension letter.
