Step 1 of 3: Choose Your Appointment Type
Choose an appointment type and click “Schedule an appointment” to continue.
- Renew (You have the option to add REAL ID during your renewal)

What documents do I need?
- Reinstate your driver's license
- Work permit (TRL)
- Add or remove restriction (i.e. SR-22)

Pay civil penalty (must have an Iowa driver's license number or customer number)
View reinstatement requirements (must have an Iowa driver's license number or customer number)
- Knowledge test (Commercial Learner's Permit (CLP), endorsements, etc.)
- Drive test (pretrip, skills, and/or road)
- Issue CDL (new, renew, restricted CDL, update Security Threat Assessment, etc.)
- Hazardous materials
- CDL medical card

How to get your persons with disabilities parking permit
Submit Your Request Electronically
- Complete the Application for Persons with Disabilities Parking Permit for Iowa Residents form.
- When prompted on the form, upload your medical statement or your US DVA certification (for veterans).
- After completing the form, electronically sign and click submit.
Go to a DMV Location
You can go to any Iowa Driver's License Service Center. No appointment necessary; however, you will need to wait for the next available team member. Additionally you can also be served by your local county treasurer's office, however, please check with their office to see if you need an appointment. Be sure to bring your completed Application for Persons with Disabilities Parking Permit for Iowa Residents and your medical statement or your US DVA certification (for veterans).
Submit Your Request by Mail
- Complete the Application for Persons with Disabilities Parking Permit for Iowa Residents form and print the form after completing.
- Mail the completed form and the medical statement to:

- International Registration Plan (IRP)
- International Fuel Tax Agreement (IFTA)
- Intrastate Travel Authority
- Prorate titles for commercial vehicles
- Oversize/Overweight permits
