Iowa driver's license or identification (ID) card name or address changes
Address changeYou must notify the Iowa DOT within 30 days when your mailing address has changed. This may be done by:
- Appearing in person at any Iowa driver's license issuance site or at one of Iowa's 24 kiosks. If your new mailing address is also your new residential address and you want that reflected on your card, you should choose one of these options. The remaining options change only the mailing address on file with the Iowa DOT, which is all that is required by Iowa law.
- By visiting our myMVD website.
There is no fee. You will not be issued a new driver's license or identification card.
- In writing by completing and mailing or faxing Change of Address Notice. Your written request must include your full name, driver's license number, new mailing address, and your signature. You will not be issued a new driver's license or identification card. Send your written request to:

Name changeChanging your name on an Iowa driver's license or ID must be done in person at any Iowa driver's license issuance site. There is a $10 charge. Applicants must provide proof of any name change.
Photocopies and faxes of documents will NOT be accepted.
A certified copy is one that contains the original seal and certificate of the court or registrar, rather than a simple file or date stamp.
