Lost or Stolen Iowa Driver's Licenses

If your license has been lost or stolen, you may apply for a duplicate.  The fee will be $3.  To protect your identity and to ensure that a duplicate is issued only to you, two forms of identification are required.  Please review Proof of Identity  for a list of primary and secondary documents.  If your photograph was stored digitally as part of your last license application, driver license personnel may consider it as satisfying one of the document requirements.  However, driver license personnel will use discretion on the acceptance and use of the photograph.

While Out-of-State
If your license has been lost or stolen and you are unable to apply for a duplicate because you are out-of-state, you may apply for a temporary driving permit. 
You must make an application in writing and include your driver's license or social security number, full name and date of birth.  You must also provide an out-of-state address and/or out-of-state fax number.  Please note that a temporary driving permit is not valid for identification purposes.
