License Name or Address Changes
Address Change
You must notify the Iowa Department of Transportation
within 30 days when your mailing address has changed. This may be done by:
- Appearing in person at any Iowa
driver's license station. You will be issued a new driver's license or
identification for $1.
- In writing by completing and mailing or
faxing
Change of Address Notice Form #430012. Your written request must
include your full name, driver's license number, new mailing address, and your signature.
You will not be issued a new driver's license or identification. Send your written request

Name Change
Changing your name on an Iowa driver's license or ID must
be done in person at any Iowa driver's license station.
There is a $1 charge. Applicants must provide proof of any name change.
Photocopies and faxes of documents will NOT be accepted:
Acceptable items are:
- Certified copy of a court order that
grants and orders the name change.
- Certified decree of divorce that grants
and orders the name change.
- Certified copy of a marriage certificate
that reflects the name change.
A certified copy is one that contains the
original seal and certificate of the court or registrar, rather than a
simple file or date stamp.
