How to Search for a Form

Forms are available in the following formats – PDF, Word, Excel, and eForms:
PDF – You can open these forms using Adobe Acrobat Reader and print them on your printer. Fill them out by hand and then mail them to the DOT. Click here to download the free Adobe AcrobatTM Reader. |

Word – Most of our Word forms are designed so you can fill them out on-line. |

Excel – Most of our Excel forms are also fill-able on-line. |

eForms – Similar to fill-able Word forms, eForms offer many other features that make it easy for you to fill out the form simply through your internet browser. eForms may also be printed and filled by hand or e-mailed to a DOT representative assigned to your project. |
When you choose “Search” on the toolbar of the Forms page, this Search Templates dialog box appears.
Enter the search criteria in the fields. To reduce the number of matches that are returned, enter more criteria. For example, if you know the title and the template ID, enter both. The search criteria are not case-sensitive; you can use upper or lowercase letters in your search.

Practical Examples: To find Form 020111 “Application for Public Transit Training Fellowship”, you can search in the following ways:
- you can enter “Application”, or “Public Transit”, or “Training Fellowship” or any other words or phrases you know are in the title of the form in the “Title” or “Description” fields and click “Search”. All forms with any of these words in their title will be displayed and in all available formats, in this case, PDF and Word.
- you can enter “Application for Public Transit Training Fellowship” in the “Title” field and click “Search”. Only this form will be displayed, showing the PDF and Word formats.
- you can enter “020111” in the “Template ID” field and click “Search”. Only this form will be displayed and in all available formats, in this case, PDF and Word.
- you may search using any of the methods above and also choose “Other” from the “Type” pull-down menu before clicking “Search”. The display will then show only Word or Excel formats which meet your other criteria. Choosing “PDF Template” or “Informed Template” work the same way in searching for only PDF or eForm formats.
